Vitamin D deficiency in Bengaluru: why it is so common and how to test
IT workers, women, and the elderly in Bangalore are routinely vitamin D deficient. Here is how to test, interpret, and supplement.
Bengaluru has plenty of sun — and one of the highest rates of vitamin D deficiency in urban India. Surveys of IT-sector adults have found 70-90% with 25-OH vitamin D levels below 30 ng/mL. The reasons are practical, not climatic: indoor work, sunscreen, traffic-shaded commutes, and a largely vegetarian diet.
How to test
25-hydroxyvitamin D (25-OH D) is the standard test. It reflects the body's vitamin D stores over the previous 2-3 weeks. Do not confuse it with 1,25-dihydroxyvitamin D, which is rarely needed (only in specific kidney and calcium disorders).
- No fasting required.
- Stop biotin supplements 48 hours before — they interfere with the assay.
- Typical Bangalore price: ₹800 - ₹2,400. NABL-accredited labs in Koramangala, HSR, Indiranagar, Jayanagar, JP Nagar, Whitefield, and Marathahalli all offer it; home sample collection is standard.
Interpreting the result
| 25-OH D (ng/mL) | Status |
|---|---|
| < 20 | Deficient |
| 20 - 29 | Insufficient |
| 30 - 100 | Sufficient |
| > 100 | Excess (rarely seen except with high-dose supplements) |
Most Bangalore guidelines target 30-60 ng/mL.
Treatment (with your physician)
The standard regimen for deficiency in adults is cholecalciferol (D3) 60,000 IU once weekly for 8 weeks, then a maintenance dose of 1,000-2,000 IU daily or 60,000 IU monthly. Sachets and tablets cost ₹15-₹40 per 60,000 IU — extremely cheap. Calcium supplementation is added only if dietary calcium is poor or osteoporosis is documented.
Retest at 3 months. Do not retest at 6 weeks — the level is still climbing.
Why supplements without testing is risky
Vitamin D toxicity is rare but real. Cases in Bangalore clinics typically involve patients taking 60,000 IU weekly indefinitely without a baseline test. Toxicity causes high calcium, kidney stones, and confusion. A ₹1,000 test before and after a 2-3 month course pays for itself.
Lifestyle: 15 minutes of Bengaluru sun
Skin synthesises vitamin D when exposed to UVB. In Bangalore (latitude 12.9° N) UVB is sufficient year-round between 10am and 3pm. Practical advice:
- Expose face, arms, and lower legs for 15-20 minutes 3-4 times a week without sunscreen on the exposed area.
- Behind window glass does not count — UVB is blocked.
- Darker skin needs longer exposure (up to 30 minutes).
- Combine with weight-bearing exercise (a Cubbon Park walk works) for bone health.
Who should test even without symptoms
- Adults aged 50+, especially women.
- Pregnant or planning pregnancy.
- Vegetarians and vegans.
- People with osteopenia, osteoporosis, or fragility fractures.
- Patients on long-term steroids, anti-epileptics, or rifampicin.
- Anyone with persistent bone or muscle pain, unexplained fatigue, or recurrent falls.
Common Bangalore pitfalls
- Over-testing: vitamin D is not a yearly screen for healthy young adults with no risk factors.
- Under-dosing: 1,000 IU/day for 4 weeks will not correct deficiency.
- Calcium overload: routine calcium supplements alongside vitamin D are not needed if your diet has dairy or ragi-based foods (a Bangalore staple).
