The tests, in plain language
- TSH (thyroid stimulating hormone) - the single most useful thyroid test. If TSH is normal (0.4 - 4.5 mIU/L for most labs), your thyroid is almost certainly fine.
- Free T4 (FT4) - what your body actually uses. Ordered when TSH is abnormal, or in pregnancy.
- Free T3 (FT3) - added when hyperthyroidism is suspected.
- Anti-TPO antibodies - screens for Hashimoto's (autoimmune hypothyroidism). One-time test if TSH is high or you have a family history.
- Anti-Tg, TSI, thyroglobulin - specialist tests, usually only ordered by endocrinologists.
For most people the right first test is TSH alone. A "total thyroid profile" (TSH + T3 + T4) is fine as a baseline but rarely necessary for annual monitoring.

When to test
- No symptoms, no family history: not routinely required. One-time TSH after 35 is reasonable.
- Fatigue, weight change, hair loss, cold/heat intolerance: TSH first.
- Family history of thyroid disease: TSH every 2 - 3 years.
- Diagnosed hypothyroidism on levothyroxine: TSH every 6 - 12 months once stable.
- Pregnancy: TSH in first trimester (mandatory), Free T4 if TSH is abnormal.
Fasting: is it needed?
No. Thyroid tests do not require fasting. Levothyroxine should be taken after the blood draw on test day - taking it before will falsely elevate T4.
