What HbA1c measures

HbA1c reflects average blood-sugar levels over the previous 8-12 weeks. It is the percentage of haemoglobin in your red blood cells that has glucose attached. Higher average sugar = higher HbA1c.

A single HbA1c ≥ 6.5% with classical symptoms (thirst, frequent urination, weight loss) is diagnostic. Without symptoms, confirm with a repeat test.

When to test

  • Screening (no symptoms): every 3 years from age 35; from age 25 if BMI ≥ 25 plus a risk factor (family history, gestational diabetes, PCOS, hypertension, dyslipidemia, South Asian ethnicity).
  • Pre-diabetes: once a year.
  • Diabetes on stable treatment: every 3-6 months.
  • Diabetes after a treatment change: 3 months later.

Do not test HbA1c more often than every 3 months — there is nothing to see; the average has not moved.

Fasting and timing

HbA1c does not require fasting and is not affected by what you ate yesterday. Time of day does not matter. This is its big advantage over fasting glucose.

It can be falsely low in: pregnancy, recent blood loss / transfusion, hemolytic anemia, chronic kidney disease, certain hemoglobinopathies (thalassemia, sickle cell). It can be falsely high in: iron deficiency anemia. In Bangalore's largely vegetarian population, iron deficiency is common — a slightly raised HbA1c plus low ferritin is often explained by treating the anemia and retesting in 3 months.

  • Lose 5-7% of body weight if BMI is elevated.
  • 150 minutes a week of moderate activity (brisk walking counts).

  • Recheck HbA1c in 6 months.
