$## FBS vs PPBS: what is the difference?

Two tests are commonly used to screen for and monitor diabetes:

FBS (Fasting Blood Sugar): Measures your blood glucose after at least 8 hours of fasting. It reflects baseline glucose without the influence of recent food.

PPBS (Post-Prandial Blood Sugar): Measures blood glucose 2 hours after a meal. Also called PP glucose or 2-hour post-meal sugar. It shows how well your body clears glucose after eating.

Both tests are often ordered together for a more complete picture. A third option, the Oral Glucose Tolerance Test (OGTT), involves drinking a glucose solution and testing at intervals -- used primarily for diagnosing gestational diabetes.

Reference ranges (standard):

FBS:

  • Below 100 mg/dL -- Normal
  • 100 to 125 mg/dL -- Pre-diabetes (impaired fasting glucose)
  • 126 mg/dL and above (on two occasions) -- Diabetes

PPBS:

  • Below 140 mg/dL -- Normal
  • 140 to 199 mg/dL -- Pre-diabetes (impaired glucose tolerance)
  • 200 mg/dL and above -- Diabetes

Fasting requirements

FBS: Fast for at least 8 hours before the test. Water is allowed. No medication changes unless instructed by your doctor.

PPBS: Eat a normal meal, then go to the lab exactly 2 hours after your first bite. Do not snack in between.

HbA1c: No fasting required. Can be done at any time.
